AstraZeneca, Cardiol Therapeutics, Kiniksa – New hope for heart patients

Cardiovascular diseases remain among the greatest medical challenges worldwide. While established pharmaceutical groups invest billions in new active ingredients and artificial intelligence, specialised biotech companies are developing targeted therapies for conditions that have so far been difficult to treat, such as recurrent pericarditis. Patients often suffer from painful relapses and require long-term treatment. New medicines could fundamentally change how these conditions are managed. For investors, this brings into focus companies that already market approved products or are awaiting pivotal clinical trial results.

AstraZeneca: AI Revolution in Drug Development

AstraZeneca is increasingly relying on artificial intelligence in the development of new medicines. The British pharmaceutical group announced a strategic collaboration with US biotechnology company insitro. Together, the partners aim to develop new drugs for hard-to-treat heart muscle and liver diseases.

AstraZeneca will initially pay USD 40 million. Successful development and commercialisation steps could trigger further milestone payments of up to USD 1.46 billion. Sales-based royalties have also been agreed.

At the centre is insitro's AI platform "Virtual Human". It combines extensive human and cellular data with machine learning to identify previously unknown disease mechanisms and suitable drug targets. AstraZeneca will subsequently take over clinical development.

The group is one of the world's largest pharmaceutical companies and is active in areas including oncology, cardiovascular diseases and rare diseases. Management sees significant unmet medical need, particularly in the cardiovascular field.

The new partnership could help shorten development times and identify promising drug candidates earlier. However, the joint programmes are still at an early research stage. It remains uncertain whether they will lead to approved medicines. In the long term, combining AI with drug development could create additional growth opportunities.

Cardiol Therapeutics: Approaching a Decisive Phase

Cardiol Therapeutics is approaching a significant milestone. The Canadian biotech company is developing CardiolRx™, an orally administered drug candidate for recurrent pericarditis. In the pivotal Phase III MAVERIC trial, the company reached its original recruitment target of 110 patients ahead of schedule. Following a blinded interim analysis showing fewer disease recurrences than expected, the steering committee recommended increasing the number of participants to up to 150. This is intended to safeguard the trial's statistical power. Results are now expected in mid-2027.

Exercising existing warrants provided additional financial security. This enabled Cardiol to raise more than USD 7 million in September. Of the original 5.71 million warrants, only 425,000 remained unexercised on September 15. The company shortened their term to October 16. According to the company's statement on September 16, the additional funds extend its cash runway into the first half of 2028, beyond the expected trial readout.

Analysts at First Berlin Equity Research are optimistic. Analyst Christian Orquera reaffirmed his Buy recommendation on October 2 and, with a price target of USD 7.40, sees implied upside of over 580% from the current share price. He describes the low recurrence rate as encouraging. One possible explanation, he suggests, is the effect of CardiolRx™. However, the trial data, which remain blinded, do not yet allow conclusions to be drawn about the drug's actual efficacy.

First Berlin views the trial expansion as a sensible measure to safeguard statistical power. Because of the longer development timeline and dilution from the exercised warrants, Orquera lowered its price target slightly from USD 7.50 to USD 7.40.

    B. Riley Securities is even more optimistic. Analyst Mayank Mamtani initiated coverage in late August with a Buy rating and a price target of USD 11. If MAVERIC delivers convincing results, CardiolRx™ could enter an already established treatment market as an oral therapy.

Kiniksa Pharmaceuticals: Heart Medicine Becomes a Revenue Driver

Kiniksa Pharmaceuticals is already demonstrating the commercial potential of treating recurrent pericarditis. With Arcalyst, the biopharmaceutical company markets a medicine approved in the US for the condition and has seen demand rise sharply for years.

In the second quarter of 2026, Arcalyst product revenue reached USD 243.6 million. This represents growth of around 55% compared with the same period last year. Given the strong performance, Kiniksa raised its full-year revenue guidance to between USD 980 million and USD 995 million.

The group focuses on innovative therapies for diseases with high unmet medical need. Arcalyst is injected once a week and inhibits the action of pro-inflammatory interleukin-1 signalling pathways.

Kiniksa is already working on the next generation. The drug candidate KPL-387 showed a rapid and sustained reduction in a Phase II trial in pain and signs of inflammation. A pivotal Phase III trial is already underway. The company is aiming for a therapy that requires injection under the skin only once a month.

This could help Kiniksa further expand its position in the pericarditis medicines market. The company is targeting a market launch in 2028 or 2029, provided clinical development is successful.

Strong Arcalyst revenues show that recurrent pericarditis treatment is already a significant commercial market. At the same time, new drug development highlights how intense competition is becoming for more effective and convenient therapies. Kiniksa Pharmaceuticals' market capitalisation stands at USD 6.13 billion, while Cardiol Therapeutics' is around USD 133 million.

    Conclusion

AstraZeneca is investing in AI-assisted drug discovery, while Kiniksa Pharmaceuticals is already generating substantial revenue with Arcalyst in the market for recurrent pericarditis. Cardiol Therapeutics, by contrast, faces the decisive clinical test. The Phase III MAVERIC trial could show in mid-2027 whether CardiolRx™ offers a new treatment option as an oral therapy. The Buy recommendations from First Berlin and B. Riley underscore the potential anticipated by analysts – with correspondingly high clinical risks.

The storm in the pharmaceutical sector continues to gain momentum! Expiring patents and struggling healthcare systems are shaking up old, high-margin blockbuster drugs, as societal changes and government budget constraints shake the foundations of costly pharmaceutical development. As patents expire, many generic manufacturers find themselves in a race against time. Market shares are being reshuffled, heralding a new era of growth. Amid this dynamic rally, four well-known industry players are positioning themselves on different fronts to capitalize on the market shifts. After several difficult years, the Leverkusen-based pharmaceutical giant Bayer is fighting its way back with a completely revamped pipeline. Also in the fast lane is the German biosimilar hopeful from Munich, with its first products already on the market. At Hamburg-based drug discovery company Evotec, restructuring is still underway; the key driver is its state-of-the-art, AI-powered multivalent platform. Meanwhile, the Danish giant Novo Nordisk continues to ride the global obesity and diabetes wave, driving forward the next generation of its blockbuster drugs. Analysts do not have it easy, but there are plenty of opportunities for investors.
