After 600% Gains with Moderna! Cardiol Therapeutics Poised for a Revaluation - Analysts See Significant Upside Potential
With gains of more than 600%, Moderna is the Nasdaq’s best-performing stock so far this year. Much of its future growth potential may already be priced in, prompting Citigroup to recommend selling shares in the mRNA specialist. Analysts, however, see significant upside potential in another biotech company: Cardiol Therapeutics. The Canadian biotechnology company focuses on inflammatory heart diseases, and its lead drug candidate, CardiolRx™, is already in Phase 3 clinical trials. Following strong Phase 2 data, the stock more than doubled between July and September. After a subsequent pullback, could this present an attractive entry opportunity? In a recent interview, the CEO explains why he remains optimistic. Two research firms recommend buying the stock and see the potential for a multiple-fold increase in its share price.

Cardiol Shares in Focus Since the Summer Following Strong Study Data
While Moderna made headlines worldwide with its personalised mRNA cancer vaccine for melanoma, Cardiol Therapeutics also attracted increased attention from biotech investors in its wake. The Canadian company's shares rose more than 100% on NASDAQ between early July and early September to around USD 2.30. Cardiol now also has a drug candidate, CardiolRx™, in Phase 3 of clinical development. CardiolRx™ is based on cannabidiol (CBD) and is intended to combat inflammatory processes in the heart without suppressing the immune system. The focus is on treating recurrent pericarditis, a recurring inflammation of the sac surrounding the heart, which can be associated with severe chest pain and significant impairment of quality of life. The publication of positive Phase 2 data provided a boost in the summer. The results showed a marked reduction in pain and inflammatory markers. The frequency of further flare-ups also decreased. The ongoing pivotal Phase 3 study is now intended to demonstrate that CardiolRx™ can actually prevent relapses.

An Exciting Market
Recurrent pericarditis is an inflammatory heart condition in which the sac surrounding the heart repeatedly becomes inflamed. Those affected can suffer from severe chest pain, shortness of breath and repeated flare-ups for years. According to Cardiol Therapeutics, around 38,000 people are affected annually in the US alone. The market potential for new therapies is correspondingly attractive. Kiniksa Pharmaceuticals demonstrates that such drugs can generate high revenues. The company aims to generate up to USD 995 million in revenue from its drug Arcalyst this year. However, Arcalyst is also used for other rare diseases. In addition, the drug must be injected weekly and affects the immune system. CardiolRx™ could gain a competitive advantage through its oral administration and different mechanism of action. If it secures approval, Cardiol could tap into an attractive pharmaceutical market.
Is the Pullback a Buying Opportunity?
In recent weeks, Cardiol shares have come under significant pressure and are currently trading at USD 1.23. The CEO himself explains why this could be an attractive entry opportunity. Analysts also see it as a buying opportunity. But first, the background. Setbacks at Novo Nordisk caused uncertainty in the sector. In a Phase 3 study, the Danish pharmaceutical group failed to achieve the targeted reduction in major cardiovascular events with its anti-inflammatory drug ziltivekimab. In early September, two further studies were terminated prematurely. As a result, other biotech companies researching anti-inflammatory therapies were also tarred with the same brush. Yet some of these companies are pursuing entirely different mechanisms of action from Novo Nordisk. Cardiol, for example, is focusing on cannabidiol, which is intended to influence several inflammatory signalling pathways without suppressing the immune system. The failed Novo studies therefore do not allow any conclusions to be drawn about CardiolRx™'s prospects of success. But the stock market is sometimes simply irrational in the short term.
Cardiol also faced an apparent setback. The originally planned recruitment target of 110 patients for the Phase 3 study was reached ahead of schedule. However, as fewer relapses than expected have occurred in the study so far, the number of patients is to be increased to up to 150 as a precaution. Cardiol thereby aims to ensure that sufficient data are available to provide statistically robust evidence of efficacy. This delays the publication of the results by around three months to mid-2027. As is well known, the stock market does not like surprises. But given the limited delay, reason could soon prevail among investors again, particularly as Cardiol CEO David Elsley even sees positive aspects in both developments in his latest interview.

CEO Highlights the Opportunities in Interview
In an interview with Lyndsay Malchuk from IIF, Elsley explains why he views Novo Nordisk's recent setbacks as an opportunity. While the Danish pharmaceutical group's drug specifically targets the immune system, Cardiol is pursuing a different approach. CardiolRx™ is intended to combat inflammation without suppressing immune defences. Elsley therefore expects the pharmaceutical industry to increasingly seek such alternatives in future. This could make Cardiol a potential acquisition target. With a market capitalisation of just over CAD 200 million, the company could command a substantial takeover premium. The CEO also views the expansion of the company's own Phase 3 study positively. The unexpectedly low number of relapses so far could indicate that CardiolRx™ is more effective than originally assumed. As the study remains blinded, a lower relapse rate in the placebo group could also be responsible. The additional patients are intended to ensure that the treatment effect can be demonstrated with statistical reliability. According to Elsley, this will increase study costs by only around 2%.
What is next for Cardiol? By the end of 2026, up to 150 patients are expected to be enrolled in the MAVERIC trial, with the pivotal results anticipated in mid-2027. If the trial is successful, CardiolRx™ could eventually be used as a second-line treatment for patients who do not respond adequately to conventional medications or cannot tolerate them. And Cardiol has another iron in the fire. The company is developing a variant of its active substance to treat heart failure, which would be injected under the skin once a month. Clinical development is scheduled to begin in 2027. In the long term, Elsley sees even greater market potential in this program. In his view, positive Phase 3 results with CardiolRx™ could also spark interest among major pharmaceutical companies seeking to establish a development partnership.
Analysts See Potential for a Several-Fold Increase
Analysts are also taking the recent decline in Cardiol Therapeutics' share price in their stride. B. Riley Securities examined the Phase 3 trial progress more closely and reaffirmed its buy recommendation, setting a price target of USD 11 on October 9. Analyst Mayank Mamtani considers the increase in patient numbers a sensible precaution to safeguard the statistical power of the trial. He points to the successful RHAPSODY Phase 3 trial of the competing drug Arcalyst. There, too, the placebo group recurrence rate was below initial expectations. Nevertheless, highly significant efficacy was demonstrated. For CardiolRx™, a relative reduction in recurrence risk of just 40% is sufficient to meet the trial's objective. If the active substance performs even better than expected, the expanded trial could provide particularly convincing evidence of the effect. B. Riley sees CardiolRx™'s substantial revenue potential as far from reflected in the current market capitalisation of around USD 150 million. The analyst believes Cardiol shares could rise to USD 11.
In their valuation model, First Berlin's analysts forecast that CardiolRx™ has total long-term peak revenue potential of around USD 2 billion across several indications. The analysts reaffirmed the buy recommendation in early October. The price target is USD 7.40. The analysts share the CEO's view that the unexpectedly low number of disease recurrences could be attributable to greater efficacy of CardiolRx™. In the analyst's view, historical comparative data support this. First Berlin also assesses the financial situation positively. Cardiol recently raised more than USD 7 million, securing its funding through the first quarter of 2028—well beyond the release of the pivotal Phase 3 data.
Conclusion: Good Reasons for a Rising Share Price
Cardiol shares are currently trading at USD 1.23, after reaching USD 2.30 in September. Analysts have set price targets of USD 7.40 and USD 11. The recent pullback could be an attractive buying opportunity for several reasons. The CEO and analysts have presented a convincing case for the company’s prospects. Should the low recurrence rate indeed prove to be an indication of high efficacy of CardiolRx™, the shares could be poised for a re-rating. Expectations of a partnership with a major pharmaceutical company could also gain traction.
